NPI Lens provides public federal data “as is”, for information. It can be out of date between monthly updates and may contain errors made at the source. It is not a substitute for primary-source verification: before credentialing, hiring, contracting or billing, confirm the record in the official NPI Registry and theOIG exclusions database.

A “possible” OIG match only means the names and state are the same; it is not a finding that a person or business is excluded. Do not use this site to make claims about anyone without confirming them.
