About license numbers in NPPES
When providers apply for or update their NPI, they list each taxonomy (specialty) with the state license that goes with it. Those numbers are self-reported and are not verified by CMS, so a license can be missing, mistyped or out of date. To confirm that a license is active, use the issuing state board's verification service.
