NPPES public data file (CMS)

The National Plan and Provider Enumeration System file that CMS publishes monthly under the Freedom of Information Act: every NPI ever issued, with names, taxonomies, license numbers, practice locations, enumeration and update dates, and deactivations.

We show the practice location and never the mailing address. Fields CMS redacts in the public file (such as EIN) are not available to us.

OIG List of Excluded Individuals/Entities (HHS-OIG)

The full current exclusion list, refreshed monthly by OIG. We match it to NPPES by NPI when the OIG record carries one, and otherwise by exact name and state — shown as a possible match, because many OIG records have no NPI and names repeat.

These files are refreshed when CMS publishes a new version (Open Payments once a year, enrollment and opt-out monthly). CMS suppresses counts under 11 to protect patient privacy.

Care Compare (CMS)

Medical school, graduation year, Medicare specialty, telehealth, medical group membership and hospital affiliations come from the Care Compare Doctors and Clinicians national file (Oct 2, 2026), the facility affiliation file and Hospital General Information on data.cms.gov/provider-data, plus the general information files for nursing homes, home health agencies, hospices, dialysis facilities, inpatient rehabilitation facilities and long-term care hospitals. They cover clinicians enrolled in Medicare.

Taxonomy descriptions

Specialty names are the descriptions the official NPI Registry shows for each provider taxonomy code, with groupings from the CMS “Medicare Provider and Supplier Taxonomy Crosswalk” on data.cms.gov.

Update schedule

We rebuild the database each month after CMS publishes a new NPPES file, and pick up the latest OIG list at the same time. Changes a provider makes in NPPES appear here after the next monthly file. Last build: Nov 4, 2026.

NPI Lens is not affiliated with CMS, HHS or OIG.
